Medicare A & B patients diagnosed with dementia are eligible to join GUIDE through The Connecticut Hospice.
Consult Your Doctor: Discuss the GUIDE program with your primary care physician or neurologist to determine if it’s a good fit for your needs.

Assessment and Care Planning: Our care team will assess your specific needs and create a personalized care plan if you're eligible.
