In addition to medical care and support, Connecticut Hospice offers patients and their families a wide variety of in-home complementary services to help patients, and their loved ones, live life to the fullest possible.
Patients, and their families, can receive complementary therapies and services in whatever setting the patient resides.
Some in-home hospice care complementary service examples include:
Our home care program is designed to meet individual needs and assist patients and their families at different stages in the course of their illness. We provide continuity of care, should illness progress and inpatient hospice care become necessary.
Our experienced, physician-led interdisciplinary team works together to provide comprehensive care to our patients and families in their own homes, assisted living and skilled nursing facilities, and other hospitals.
Our Hospice Home Care program is appropriate for patients with terminal illnesses whose life expectancy is less than six months.
In addition to Hospice Care, we offer outpatient Palliative Care for patients still undergoing curative treatment. Our "Stand by Me" palliative care program is designed to prioritize your comfort, care, and support.
Our devoted team of physicians and healthcare professionals is readily available to provide relief at our outpatient clinic, or to homebound patients in their homes, or within assisted living facilities.
We offer expert symptom management and counseling on disease management options.
As a not-for-profit, we depend on generous donors to help us provide customized services and therapies that aren’t completely covered by Medicaid, Medicare, or private insurance.
Please make a gift to help us sustain the highest standard of care.
